Impotence DISEASE

Impotence DISEASE
DEFINITION
Impotence (Erectile Dysfunction) is the inability to initiate and maintain an erection.
CAUSE
Impotence is usually the result of:
# Abnormalities of blood vessels
# Abnormalities innervations
# Drugs
# Abnormalities of the penis
# Problems that affect the psychological sexual arousal.

The cause of the physical are more common in older men, while psychological problems are more common in younger men.
Increasing age of a man, it is becoming more frequent impotence, although impotence is not part of the aging process but is the result of a disease that is often found in the elderly.
Approximately 50% of men aged 65 years and 75% of men aged 80 years of experience impotence.

In order to erect, the penis requires adequate blood flow. Therefore, blood vessel disease (e.g. atherosclerosis) can lead to impotence.
Impotence can also occur as a result of a blood clot due to surgery or blood vessels that cause the disruption of arterial blood flow to the penis.

Nerve damage leading to and leaving the penis can also cause impotence.
This nerve damage can occur due to:
# Injury
# Diabetes mellitus
# Multiple sclerosis
# Stroke
# Drugs
# Alcohol
# Lower spine disease
# Rectum or prostate surgery.

Approximately 25% of cases of impotence are caused by drugs (especially in older men who consume lots of drugs).
Drugs that can cause impotence are:
- Anti-hypertensive
- Anti-psychosis
- Anti-depressants
- Tranquilizers
- Cimetidine
- Lithium.

Sometimes impotence due to low testosterone levels.
But the decline in male hormone levels (which tend to occur due to the aging process), usually more often causes a decrease in sexual desire (libido).

Some psychological factors that may cause impotence:
# Depression
# Anxiety
# Feelings of guilt
# Feelings of fear of intimacy
# Indecision about sex.

SYMPTOMS
Sufferers are not able to initiate and maintain an erection.

Diagnosis
Diagnosis based on symptoms.
Physical examination performed to look for a change in male sexual characteristics, such as breast, testicular and penile size, as well as changes in hair, skin and voice.

To detect abnormalities in the arteries in the pelvis and groin (which supply blood to the penis), measured blood pressure in the leg.

Other tests may be conducted:
# Complete blood
# Checking blood sugar for diabetes
# Check TSH levels
# Ultrasound penis.

TREATMENT
Impotence is usually treatable without surgery.
This type of treatment depends on the cause.

Specific exercises performed by patients with impotence due to psychological problems, namely the so-called third stage sensation centering technique.
This technique encourages intimacy and emotional warmth, which is focused to build a relationship:

A. Phase I: courtship, the couples concentrate on pleasing each other without touching the pubic region.
2. Phase II: the couple began to touch the genital area or other erotic areas, but not sexual intercourse.
3. Phase III: the act of intercourse.

Each reached the comfort of intimacy at each stage before moving on to the next stage.
If this technique does not work, patients may need to undergo psychotherapy or sexual therapy.
If people are depressed, antidepressants can be given.

Sildenafil is a drug that could increase blood flow to the penis.
This drug is taken 30-60 minutes before sexual intercourse, is effective only if accompanied by sexual arousal.
Not be taken along with nitrates because it can cause serious side effects.

If impotence or loss of sexual desire due to low testosterone levels, patients should undergo hormone replacement therapy.
Testosterone injection is given every week or in the form of plaster.
Side effects of prostate enlargement and excess red blood cells which can lead to stroke.

Fastener or suction tool often used to achieve and maintain an erection, but this tool should not be used by people with bleeding disorders or patients who take anticoagulant drugs.
Fastening tool (a rope or a ring made of metal, rubber or leather) mounted at the base of the penis to slow the flow of blood from the penis.
A vacuum (in the form of a hollow box and pump) is installed on the penis. Vacuum pressure to help the drainage of blood into the arteries of the penis. When the penis is erect, a fastening device installed to prevent drainage of blood from the veins. Combination of the two devices can maintain an erection for 30 minutes.
Sometimes cause problems when the fastener means ejaculation, especially if tied too tight. For the sake of safety, preferably after 30 minutes of the tool is released.
If too much is used, a vacuum can cause bruising.

Impotence can be treated with injections of a special drug that done by the patient.
The drug is injected directly into the erectile tissue of the penis (corpus cavernous. Erection occurs within 5-10 minutes after the drug is injected and can last for 60 minutes.
The side effects are bruising and pain. In addition, the injection can also cause priapism (persistent and painful erection).

If impotence does not respond to various treatments above, can be the penis transplant, or use prosthesis (artificial penis).
One such tool is grafted a stout rod that is inserted into the penis to cause an erection that persists. Other tools in the form of a balloon inserted into the penis and inflated before the patient had sexual intercourse.
